Position:

Direct Care Provider BERNALILLO COUNTY (South Valley)

Essential Duties and Responsibilities (Performs other related duties as assigned.):

Provide home management and maintenance including light housekeeping, making beds, changing linens, wash dishes, laundry, and routine maintenance of the participant’s home.

Make minor repairs to clothing and the participant’s household furnishings. Transport client to appointments if required by individual service plan and perform errands and shopping necessary for the maintenance of the participant’s home.  Provide assistance with personal hygiene to assure comfort, cleanliness, and appearance of the participant. Provide assistance with participant’s mobility to and from bed to chair, wheelchair, bathroom, or eating area. Assist in meal preparations and eating as determined by Individual Service Plan or Personal Care Service Plan. Assisting with self-administered medications, prompting and reminding. Observe the participant and promptly report significant changes to the Director of Homecare Services or Service Coordinator.  All other observation should be documented bi-weekly and submitted with paperwork.

Hard Criteria:

High School diploma or GED preferred; direct care experience with the disabled and/or elderly a plus.  English/Spanish bi-lingual a plus.  Must provide current valid NM Drivers’ License and Proof of Auto insurance.

Soft Criteria:

Good general health with the ability to lift 50 lbs. and unlimited range of motion.  Able to drive vehicle.  Requires excellent organizational and interpersonal skills.
