Service Fee Schedule:

Hourly Caregivers………………………………………………………..…… $16/hour

• Requires a 2-hour minimum shift during daytime hours (6AM to 6 PM) and a 4-hour minimum shift during evening hours (6PM to 10 PM).  Hours of service provided beyond the minimum shift are billed at 0.25 hour increments.
• When caring for 2 persons per household………………………….... add $2/hour

Additional Fees & Details:

• Transportation reimbursement………………………………… 40.5 cents/mile

 A transportation fee is assessed when transportation on errands & to appointments are provided utilizing the Caregiver’s vehicle.

• Initial In-home Consultation………………………………….…complimentary

• Service Needs Assessment, Periodic Re-assessments………..…complimentary

A Service Needs Assessment is completed when you choose La Vida Felicidad for Caregiver services.  Re-assessment is performed annually thereafter or more frequently if a significant change in the client’s needs occurs or is indicated.

• Service Plan development and on-going Service Plan Review….........................................................................................complimentary

A Service Plan is developed prior to delivery of services and outlines all services to be delivered.  This Plan is reviewed at least every three months or more frequently if a significant change in the client’s needs occurs or is indicated.

• Major Holidays are billed at time and one-half when shift begins during that holiday’s 24-hour period
• Rates are subject to change with 30 day written notice
• A refundable service deposit is required prior to the start of service
