Developmental Disability Medicaid Waiver Program

The purpose of the Developmental Disability Medicaid Waiver Program (DD Waiver) is to support people with developmental disabilities. DD Waiver funds pay for an array of services. These services provide support, which allow people with disabilities to live in their community among family and friends.

La Vida Felicidad, Inc. provides the following DD Waiver services to eligible individuals at no cost to families:

Personal care
Respite care
Home Based services
Community Membership
Day habilitation programming (see Adult Activity Program for day habilitation services)
Assisted Living

Eligibility

An individual or family applying for DD Waiver services in New Mexico must meet the State and Federal Definition of Developmental Disability per the Long Term Services Division of the Department of Health

You may also contact the Metro Office in Albuquerque, or go to the regional office and ask for a Request for Services packet
Request for Services packets and ISD applications are also available at our agency office in Los Lunas
