Disabled and Elderly Medicaid Waiver Program
The purpose of The Disabled and Elderly Medicaid Waiver Program is to support elderly and/or disabled individuals in their homes. Services provide support, which allow people with disabilities to live in their community among family and friends.
La Vida Felicidad, Inc. provides Homemaker/Companion services to individuals eligible for the Disabled and Elderly Waiver.
Eligibility
- Elderly, blind, or disabled as defined by the Social Security Administration
- Individuals must require a Level of Care (LOC) provided by a nursing home
- Individual must meet income eligibility
Services
- General household services and support necessary to maintain the home (or living area occupied by the recipient) in a clean and safe environment
- Meal preparation and assistance
- Shopping and/or errands; escort services
- Homemaker/Companion services are NON-MEDICAL services
- Assisting with self-administered medication
